JFS at Home is dedicated to helping seniors, adults with illnesses, and people recovering from surgery to live independently in a safe, comfortable, and supportive setting. Independence, however, doesn’t mean having to do it all alone. Whether you need a little or a lot, JFS at Home can help you or your loved one.

In-Home Assessment
You are not like anyone else. Neither are your needs for care. That’s why JFS at Home starts the care coordination process with a personalized intake to ensure we understand all that is needed. When you call, our intake specialist will listen to your concerns and work with you, your loved one, and family to evaluate available options.

Together we will examine:

• Physical and emotional health
• Social activities
• Home safety and maintenance needs
• Needs for transportation, housekeeping, meal preparation, running errands,
  bathing, dressing, etc.
• Financial resources
• Emergency contacts

Plan of Care
Once the in-home assessment is completed, JFS at Home will prepare a personalized plan of care. This plan, which we will review together:

• Outlines specific areas of concern we have identified
• Recommends actions that will address the plan of care
• Establishes a time frame for implementing the plan of care

Your Caregiver is Our Employee
JFS at Home ensures that all employer responsibilities are handled including:

• Taxes, insurance, benefits, and worker’s compensation
• Training and supervision
• Complete background check
